◆How to Complete Your Application

Please do the following to complete your application:
1. Click the link of Online Application and enter your information on a website.
2. Print your application form.
3. Sign the application form.
4. Send the signed application form by FAX to SEMI Japan at the following number: + 81-3-3222-5757

Your application must be received by SEMI Japan within one week of entry information on a website. An acceptance notice and invoice will be sent upon receipt of your application form, at which time your application will be considered complete.
